Is your patient at high risk of developing type 2 diabetes or do they have prediabetes (HbA1c 5.7-6.4%)? At 12 months post program, participants maintained an 8.2lb weight loss, 4.6cm waist circumference reduction, and a 0.19% HbA1C decrease.

Small Steps for Big ChangES is a program that can empower your patients to make lasting dietary and exercise changes to lower their risk of type 2 diabetes. 

 

The program was developed and tested by the Diabetes Prevention Research Group at the University of British Columbia. We found that most individuals continued to be physically active and consumed a healthier diet months after the program ended. Those who participated in the program also improved their fitness levels, decreased their waist circumference and blood pressure, and experienced weight loss.

WHO IS ELIGIBLE?

Your patients may be able to participate in the Program if they are: 

  • 18 or older

  • At risk for type 2 diabetes
